The incident occurred in the morning hours on the Warsaw–Lublin route, when a passenger train driver reported irregularities on the track near the village of Mika in the Masovian Voivodeship. Inspectors who arrived at the site found that a section of the rails and track bed had been severely damaged by an explosive device. Later that day, a second damaged location was discovered further along the same line.
